Talk to Abby's Friend

Right at the beginning, before the cutscene you will obtain the trophy/achievement Heart of the Serpent for finding the Mission of San Juan. After the cutscene you will obtain +500 XP. Follow Abby across the square of San Juan and down a set of stairs to the right where you meet Sara.
Sara points Lara to the library.
You are rewarded with +500 XP.

Go to the Library

Mission "Star Crossed: Explore the Local Mysteries" (1): Speak to the Guillermo standing to the left of Sara to begin another mission. [Screenshot] He asks you to find their missing colleague, Isabela.
Investigate the Cemetery
Head up the stairs again.
Conversation - Point of Interest (1): Talk to the man standing at the corner at the top of the stairs from where the two archaeologists are. Inspired by his father he is a hobby archaeologist. He will reveal the location of a monolith to Lara.
Turn right.
Merchant: At the stalls under the red tarp you can find the local merchant. Speak to her to sell all your gold and jade ore and any resource that might be filling up your inventory to make room for new stuff. You can also buy the following items here: WHWS-16 rifle (5800), TAEV 16 shotgun (5700), WASP 11 rifle (5900), Rifle Suppressor (3400), Shotgun Spreader Choke (3300), Shell Holder (2900)
Continue through the damaged arch to the square in front of the church.
Conversation (Optional): When entering the square in front of the church, coming from the entrance square, talk to the woman leaning against the pillar on the left. She will reveal something about the history of the mission.
Conversation (Optional): Speak to the child sitting in front of the jeep on the right side by the church. He will warn you of a witch at the graveyard.
Continue straight and through the arch at the end, where Jonah is waiting, to reach a courtyard in front of the cemetery. Follow Jonah and obtain the pamphlet Seven Steps Closer to God from the collection "Miracle Mission" (1/8). Jonah makes his way to a door on the left, but we will continue to the right. Should you get too close to the door, Lara might follow Jonah in, gain 50 XP and get the next objective. In that case just exit again.
Conversation (Optional): Talk to the woman standing on the right path, to the left of a small tree offering hardwood, who will speak about the graveyard.
Relic (1/9): Follow the right cemetery path further to the end where it bends to the left. At a grave straight ahead you will find a small chest containing the relic Slave Collar from the artifact collection "Conquerors" (7/8). It contains hidden information. Take a closer look at the inside near the clasp. You are rewarded with +25 XP. [Screenshot], [Screenshot] (map location)
Archivist Map (1): Sneak through the vegetation behind the grave to find an Archivist Map in the southern corner of the cemetery. It will add the location of several documents to your map and comes with a reward of +25 XP. To the left of the map you can find a resource container with +15 XP. [Screenshot], [Screenshot] (map location)
Now find an old woman by a tipped grave marker on the path. [Screenshot], [Screenshot] (map location) Speak to her to find out that Isabela was the one who tipped over the cross when trying to climb on it. Lara offers to help her to erect the cross again but the woman tells her that they have to get rid of the children first.
Convince the Children to Move
Speak to the boy in the grey shirt, whose name - as we will later learn - is Diego, on the left. [Screenshot] He reveals that they are hunting for a treasure. Lara promises to help them with their search.
Find the Hidden Treasure
The children will have given Lara a riddle. I want you to take my supply. I'm nourished when the heavens cry. If I get sick a village dies. Since the riddle is solvable within the confines of the cemetery, the river can not be the answer, but there is a well at the south-western side of the cemetery. [Screenshot], [Screenshot] (map location) Get behind it, to dig up the treasure. You will obtain +25 XP as well as the fools gold and fake jewellery found inside.
Return to the Children
Make your way back to the tipped cross and speak to Diego, the boy in the grey shirt, again, to hand the treasure to the children. [Screenshot]
Return to the Woman
Now that the children are gone, talk to the old woman again to re-erect the cross. [Screenshot]
Examine the Cross
Examine the cross. Lara will find an inscription Look through me to find the way. You will obtain +100 XP.
Follow the Path of Crosses
Looking through the circle of the cross you can spot another cross up on the arch above the entrance to the cemetery and two left and right of the arch. [Screenshot], [Screenshot] Run over there and step through into the courtyard in front of the cemetery. To the left of the next stone arch on the left and through it you can spot two more crosses. [Screenshot] Step through the arch and spot the next cross by the ruin that is being renovated on the right. [Screenshot] From there look over to the next cross at the far end of the square, on the left side of the path, near the jeep. [Screenshot] Stand beside the jeep and look down the stairs on the left. In front of the vegetation you can spot another cross. [Screenshot] Before you head down there, grab the salvage on the right wall. Once you have reached the next cross, look down to the right, past Sara and Guillermo to spot the next cross at the river bank. [Screenshot] The next cross is found on top of the bridge across the river. [Screenshot] Stand near the tree stump wrapped in rope and tether it to the pole just to the right of the many-faced stone. Climb over. The next cross ist just across the road. [Screenshot] Grab the perception plant to the left of the path heading down hill on the other side of the road. Below, in the far distance you can spot another cross. [Screenshot] Make your way downhill.
Relic (2/9): Over on the left side you can find a small altar with a cross. Get over there to find a small box with the relic Faded Writing from the artifact collection "The Missionary" (6/8). You are rewarded with +25 XP. [Screenshot], [Screenshot], [Screenshot] (map location)
Jump across the crevice to the other side. On the left side you can pick a perception plant. Shortly after you will find some medicine on the left and an endurance plant on the right.
Survival Cache (1/8): On the left is a cave opening. Step inside and get the bow (or, if you are bad at aiming, the shotgun) ready. Kill the four wolves, one black and three of them red. Loot them. The black one offers rare hide. Then explore the cave. Not far inside, in front of the pillar, by a pile of bones you can dig up a Survival Cache and earn +25 XP. Further at the back, on the left wall you can find a vein with gold ore (+6). Exit the cave again. [Screenshot], [Screenshot], [Screenshot] (map location)
Head further down towards the cross.
Investigate the Final Cross
Push the cross on the stone marker. [Screenshot] A stone door at the overgrown ruins at the back will open.
Find Isabela
Collect the resource container with +15 XP behind the cross on the right, then make your way to the opened entrance and step inside.
Crypt (1/1) "Huascar, the Syncretist":
Document (1/27): Immediately on the left you will be able to collect the document The Chosen One from the artifact collection "Cult of de la Cruz" (1/7). You will obtain +25 XP. [Screenshot], [Screenshot] (map location)
Step towards the edge and jump to the craggy wall in front of you. Rappel down then drop the last meters. Follow the dark tunnel into the crypt. You come by an endurance plant before you reach a deep shaft. From below you can hear Isabela answering Lara's calls. Jump towards the next craggy wall across the pitfall and climb down. From the lowest end rappel down, ignoring the craggy wall leading to the right, which forms the way back up. At the end of the rope drop down to land in a pool of water.
Resource: Dive down to find a vein of Jade Ore (+6) in the south-eastern wall, before returning to the surface.
Speak to Isabela
Climb out to find Isabela sitting to the right of a sarcophagus. [Screenshot] Speak to Isabela to learn that she injured herself when the tremor hit and that she needs help with getting out of the crypt. You will earn XP.
Exit the Ruins
Collect the cloth found nearby, then open the Sarcophagus. You will obtain +125 XP, +750 gold pieces and the Vestige: Kantu's Gilded Vest. Now find the low opening in the left (south-east) wall and crawl through it. At the T-junction ahead you can collect a resource container for +15 XP.
Mural (1/4): Turn around and look at the wall to the right of the opening you came through to spot the mural Huascar, the Syncretist from the artifact collection "Resting Places" (6/8). It will improve your knowledge of Quechua and reward you with +35 XP. [Screenshot], [Screenshot] (map location)
Document (2/27): From the entrance where you crawled in turn right (south-west) to where you see a collapsible floor with spikes hidden underneath. Jump across to the other side and enter the chamber. On the stone slab at the back wall you will find the document Believer's Journal from the artifact collection "Cult of de la Cruz" (2/7). You will be rewarded with +25 XP. [Screenshot], [Screenshot] (map location)
Resource: Use the shotgun to shoot through the wall on the right of the document. In the catacombs behind you will find a resource container with +15 XP in a niche to the left and some Jade Ore (+6) in the alcove with the wooden poles through which you can see the main chamber with the sarcophagus. Turn back and jump across the spike pit to return to where you found the mural.
Beyond the mural you can squeeze through a gap in the east wall.
Document (3/27): A few steps along, by a skeleton on the left side, you can find the document Hubris from the artifact collection "Cult of de la Cruz" (3/7). You will obtain +25 XP. [Screenshot], [Screenshot], [Screenshot] (map location)
Follow the tunnel to the large spear pit. Use the grapple axe on the grapple point above you to swing to the other side. Depending on your rope swinging skills you may either have to attach to the craggy wall or be able to grab the ledge. Pull up. Follow the tunnel past another endurance plant.
Document (4/27): On a stone altar on the left you will find the document Pure of Heart from the artifact collection "Cult of de la Cruz" (4/7). Again you are rewarded with +25 XP. [Screenshot], [Screenshot] (map location)
Head over into the right corner where you will find another craggy wall you can climb up. Scramble up the wall to reach an area where fresh air can enter from above.
💡
Relic (3/9): Turn to the opening on the left. Jump across the gap and grab the ledge on the other side. Pull up. Head into the lit alcove on the right to find a small box containing the relic Wheel-Lock Pistol from the artifact collection "Angel de La Cruz" (1/7). You are rewarded with +25 XP. It contains hidden information. Take a closer look to spot the damage to the pommel. [Screenshot], [Screenshot], [Screenshot], [Screenshot] (map location) Run back to the edge and jump across the gab back to the other side.
Follow the passage further along, jumping across the spike pit hidden beneath a collapsible floor. Duck through the low tunnel and carefully advance to the next part. Where there is a Christian idol on the left wall you will find the trigger pad for a swinging spear trap on the ground. Stand back and shoot it with an arrow to disarm the trap, or trigger it and quickly destroy the trap by shooting at the wooden connector. You will gain +10 XP. Head through the arch to find a snake lever in the next chamber. Push it to open the spear gate on the left. Step into the opening and swing across the pit using the grapple axe. Hold on to the ledge on the other side and pull up. Follow the passage to the right and vault up onto the next ledge. Here use a rope arrow on the barricade on the right. Step into the opening and jump towards the craggy wall slightly to the right. This is the one where you rappelled down to reach Isabela. Climb up on the right side and at the upper end jump up to the ledge. Traverse to the right and pull out of the shaft. Run into the passage.
Return to Guillermo
Follow the tunnle to the rope ladder and climb up. Jump up where the gap prevents you from simply climbing on. Then pull up and exit the crypt.
Make your way back up the hill, jumping across the gap in the process. Head up to the dirt road and cross it to return to the pole by the many-faced stone. Use the rope slide, you might have to recreate, to return to Guillermo and Sara. Speak to Guillermo to tell him you have found Isabela. Guillermo also tells you to speak to Manu, who set Isabela on her quest and who also sent the children to the cemetery.
Talk to Manu
Head up the stairs to return to the town square. Get into the courtyard with the church. Sitting across from the church, at the edge of the cafe, with the children from the cemetery gathered around his feet, you will find Manu. [Screenshot], [Screenshot] (map location) The children will tell Manu that Lara found the treasure for them and Lara tells him that Isabela found a crypt. He will point Lara to a tomb south-east of the mission.

You need to make your way over to the entrance to the challenge tomb. The walkthrough will guide you there eventually. [continue]
Let us now take the time to fulfil challenges and grab some collectibles at the Mission of San Juan, or you can choose to skip them .

Challenge "Raise the Flag" (2/5): Head out onto the dirt road where two men are inspecting the gate. In the left corner (this is the far northern end of the mission) you will find the next flag where you can cut the release pulley to raise it and obtain +10 XP. Lara will land in another pool. Dive down into the underwater chamber. Evade the piranhas and squeeze through the gap just below the surface on the opposite wall. To the left you will see a raft that has been tied down just below the surface. Dive down to cut the rope using your knife. Swim to the surface and climb onto the raft.
